Busy Internal Medicine practice currently seeking a full-time Chronic Care Management personnel to provide chronic care services to Primary Care patients for Wellness and Disease Prevention.
Will coordinate with providers and patients to improve patient satisfaction, enhance quality patient outcomes, safety, reduce the occurrence of adverse events, and decrease hospital and emergency room admissions by performing the following job duties:
Work collaboratively with providers and their patients with chronic illnesses such as diabetes, hypertension, asthma, obesity, cardiac risk reduction and stress reduction) and promotion of wellness.
Promote positive behavioral changes to facilitate wellness and medication compliance.
Role will also include Primary Care assistant duties but not limited to, assisting the patient with prescription needs/refills; TRIAGE including telephone encounters, coordinates services with families including outside organizations.
Evaluate patient care guideline and determine areas of improvement and education.
Skill highlights required for this position include knowledge of medical terminology, medication administration understanding, collaborative nature, remarkable interpersonal skills, and on-going continuous professional development.
